I acknowledge and agree to all of the following terms and conditions:
- As a weight loss challenge participant, I agree to pay a nonrefundable prepayment of $29.00 to enter a weight loss challenge.
- I recognize that the challenge will be 12 weeks in duration.
- I recognize that the winners for the challenge will be selected on a subjective analysis and
these 5 factors will be evaluated for each participant: Dramatic difference in before and after picture, inch loss, weight loss, essay, and class attendance. Winners will not be selected solely by the inches and pounds lost, but the total transformation and participation.

Payouts will be as follows:
1st place will get 50% of prize money
2nd place-30%,
3rd place-20%
- I recognize that if I choose to drop out of the challenge, I am entitled to NO refund for any monies paid to the weight loss challenge.
- I recognize that I may choose to follow any weight loss program.
In order to be eligible to win the prize money I recognize ALL conditions below must be met:
- I agree to submit my accurate weight and measurements prior to joining a challenge.
- I agree to submit my BEFORE picture while holding a visibly dated newspaper, before the challenge begins.
- I agree to submit my AFTER picture while holding a visibly dated newspaper, the last week of the challenge.
- I agree to update my weight and measurements every week before class time.
- I agree to write a short essay on week 11, stating how the weight loss challenge has changed my life and what I have learned from the classes.
- I agree to regularly attend the weekly classes.
